1. Acceptance as member subject to approval of the Association.

本申請接納與否,由聯會決定.

2. A payment of $60 (for team /club/school) or $350 (for sponsored team) for subscription fee has to be submitted with this application.

會籍費用$60(隊/會/學校)或$350(商業贊助隊),必須與本表格一併呈交.

3. A club must upon submission of this application has at least 3 members duly paid up their own subscription fee together with insurance premium for races / activities.

屬會於呈交本表格時必須擁有最少3名已交妥會費及保險費的會員.

4. Choices of name and racing jersey colours will be registered with the Association by this application on a first-come-first-served basis, but the Association reserves the right to reject any name which might badly affect the image of sports and cycling, and any colours which will infringe the UCI regulations.

屬會名稱及賽衣顏色如有雷同,則以本表格的呈交先後作準,先到先得.唯命名如對單車及其他運動形象有損,或顏色有違反UCI規則,則本聯會有權拒絕登記.

5. The Team manager of an affiliated club will be considered as the sole duly authorized representative of the club.  If a club changes its Team manager, the notice of change must be submitted to the Association with a copy of the resolution to substantiate the validity of the change.

屬會領隊為屬會的唯一全權代理人,任何領隊人選更改必須連同一份有關決議案照會聯會以便證明該更改的有效性.

6. A club must provide the letter of authorization from the two Major sponsors to prove the club is endorsed to use the brand name(s) and the trademark(s).
   
   屬會必須提供最大兩間主要商業贊助商授權書以證明該屬隊獲授權使用贊助商名字及商標.
